Eighth St. Home

The Eighth St. home was the first group home developed in Revelstoke to address the needs of individuals reconnecting with their communities after the Tranquil Institution closure. The Eighth St. home now provides a safe, caring, home for our senior population. Up to five individuals live at this licensed home

One of the unique opportunities provided at the Eighth St. home is our training apartment. While the individual lives in the one bedroom apartment, she/he develops the skills needed to live independently with the support of staff, family and friends.

Second St. Home

The Second St.  home provides care for four adults with developmental disabilities. In response to the changing skills, abilities, and goals of the individuals the staff support the individuals to reassess their goals and objectives in order to live as independently as possible. Each person has the opportunity to experience meal prep, grocery shopping, leisure activities, house cleaning, yard maintenance, and many more typical home activities. Each participant is supported to meet their own unique goals and to be active in their community.

 Fifth St. Home

This home opened in July 1997 as a licensed home providing care for up to four individuals. In order to meet the ever changing goals of the individuals with developmental disabilities in our community, the focus of this location has changed to be a residential option as well as a meeting place for our Individualized Community Inclusion program.

Supported Employment Program

The Community Connections Supported Employment program, currently supports 11 program participants, aged 18 to 64.  Each participant is given an extensive vocational assessment, determining individual strengths and weaknesses.  We assess physical ability and endurance, problem solving skills, communication skills, memory, reading, writing, and math, not to mention what each client’s interests are, what previous job and volunteer experience they have, and what their dream job might be.

Participants are then matched with employers and a resume/job proposal is sent out.
If the employer is interested, an interview is set up, and job readiness training begins.
Pre-employment training includes learning how to appropriately and politely interact with co-workers, how to access work safely and on time for each shift, and what to expect on the first day of work.  Once employment begins and job training starts, Supported Employment Staff assist clients to break down complicated tasks, and bridge the communication barrier that can sometimes exist between the participant and employer.  Initially, Support Staff will be present at each shift, helping to form a workplace routine.  Many participants that move through the program, go on to work independently or with minimal outside support.

Individualized Community Inclusion Programs

Community Connections Individualized Community Inclusion Program operates Monday to Friday supporting up to 12 individuals.

Participants, with assistance from their support team, design an individualized plan that is tailored to their specific needs, wants and goals. Individuals access the community through educational, social and recreational activities. A few examples of favorite activities are communicating with friends and family using face book accounts, playing bingo while socializing with new friends at the Senior’s Center. We also enjoy swimming, bike riding and of course out of town field trips to meet with friends from other communities for picnics and sightseeing.

Annual activities include camping, cultivating and caring for the Community Garden, fundraising for and participating in the Relay for Life and raising community awareness every October during Community Living Month.

Weekly group lunches provide an opportunity for participants to gain knowledge, skills and confidence in meal preparation and cooking.


Educational activities such as sign language has greatly improved communication and conversation skills. Numerous educational in-services such as bike and fire safety, bear awareness, natural composting, oral hygiene care, fishing, card making, food safe, nutritional counseling, Karate, music and many more have enhanced the life experiences of our participants.  


The individualized Community Inclusion Program is dedicated to offering activities and programs that will further personal development and empower our participants to explore their dreams, goals and creativity.
